Greenwood Surgery are using a new online service called ‘Register with a GP surgery’ that makes it easy to register and quicker to access our range of services.
Just fill in the quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register. Paper forms are still available if you need one.

Documentation will be reviewed by the administration team (takes 3 working days)
Once accepted, any additional form (e.g. waiver for out of area patients) will be provided
We will arrange a 20 minute ‘health check’ where we ask you to bring a small urine sample.
Your new registration form will be sent to the Health Authority and you will receive confirmation that you are registered with us. However Medical treatment is available from the date of registration.
