Patients wishing to register for this service may download the form below and print it out. Please complete the form and return it to the surgery (in person ). You are required to bring photo identification (such as a passport) and proof of address in the form of a utility bill or some other letter for verification purposes.
SystmOnline
Greenwood Surgery’s patients have the ability to book appointments, renew prescriptions, request advice and sick notes online. This service also includes viewing parts of your Health Records. For more information, please contact reception and ask for a form to sign-up for this service.

Prescriptions
Please allow two full working days for prescriptions to be processed and remember to take weekends and bank holidays into account. Requests for medication not on repeat will first be passed to a clinician to ensure they are appropriate. Patients on long term medication can order repeat prescriptions in a number of ways. Note: We cannot accept request prescriptions either verbally or by telephone due to the risk of errors being made.
